What about the pogo effect that all the early boosters had? Could this
also have contrib. to the failure. If a pogo wave of energy trapped in
the full length of the stack is shortened, (Buy stagging, (and if not
accounted for)), then it can reform in the second stage, (shorter
travel time). This would be a good answer, (I think).
