There's not a lot (some, but not a lot) to be gained by releasing a second
stage launch vehicle from high altitude but at subsonic speed, and it
doesn't take that long to reach the maximum feasible altitude for a
subsonic flying vehicle anyway, so that refuelling hardly makes sense.

Oh come on, think of all the complications it creates that go against safety
and performance.

Haller's going to love this one since then he can rail about how unsafe it
is after the Shuttle stops flying.
