It is a requirement for all launch vehicles to have such a system and
without one, Ares I can not be flown from KSC
Not, it's not. AMROC's SET-1 vehicle didn't have one, and neither
does SpaceX Falcon 1. I don't think that Pegasus does, either. No
reusable piloted vehicle would have one. It is only a requirement on
a government range, and even there, it can be waived.
It is for KSC/CCAFS
It is, until it isn't. Elon will be attempting to get a similar
waiver there.
As for an RLV, it depends on the fuel load.

doesn't mean it automatically doesn't need one.
Of course it does. If the vehicle is designed to be so unreliable as
to need one, it would be foolish to make it either reusable, or
manned. Are you really saying that the RSO is going to destroy a
vehicle with a pilot on board? That's absurd. We might as well put
destruct devices in airliners, in that case, since they carry huge
amounts of fuel, and can fall in populated areas.

places than KWAJ since it demonstrated the need for one on its first
launch
No, it didn't necessarily. It never went out of control. It simply
fell down in the lagoon. Had it happened at the Cape, it wouldn't
have caused any damage, or at least not any that would have been
mitigated by destroying it.
The FAA will require such systems on non govt ranges.
Again, it depends. There is no blanket rule. All that has to be
proven is that it meets Ec requirements. If that can be done without
a vehicle destruct device, AST will grant a license. It's not the
sixties any more.
Waivers will be a rare event.
Waivers will not necessarily be required on non-government ranges,
since range rules don't even currently exist for them.
