Not true. The orbiter TPS failed. Debris shedding is a common
environment on all launch vehicle. There is no way of eliminating
it. The orbiter TPS can't survive the basic launch environment.
It is a design flaw in the shuttle and all the debris mitigation is
only a band aid and doesn't fix the basic problem
.
