That's because there's a major disconnect in the market between the
provider and the consumer. Health insurance has been a disaster since
the government rigged the tax system so that you can only get the
deduction for it if you're an employer.

I don't really know how the US health care systems works (or doesn't
work). But if the employer gets the deduction, presumably he, not the
beneficiary, is the consumer. Doesn't the employer have an incentive
to shop around to control health care costs?

Yes, but he doesn't have the incentive to shop around for good health
care...
