Even then, an external indication of "which way was 'up'" might have alerted
someone of a potential problem.

Of course, maybe what is wrong with NASA is that few "managers" or
"administrators" take the time to go to the trouble of entering the clear
areas and actually LOOKING at the $billion or so project they are
responsible for.

Maybe NASA could learn from the military where successful commanders when
the situation permitted would visit the lower ranks and the front lines to
get a feelling for what was really going on.
