I seem to recall a Soviet missle sub being sunk at sea
with all of its missles and wareheads, after one of the
liquid-fueled missles caught on fire in one of the
launch tubes.
Seems like the SU retired the fleet handling that class
of IRBM not too long thereafter, but that may have been due
to simple obsolescence as much as safety concerns.
