The only thing they're reusing from Apollo is the exterior shape, and even
that will be increased in size. Everything else about it will be new.

They won't "live" in the CEV for a 7 month trip to Mars, just like they
won't be living in the CEV for six month missions to the moon. For lunar
trips, they'll live in the lander, for trips to and from mars, they'll
likely live in a module (or modules) similar to space station modules. The
CEV will only be used for launch to LEO and for reentry when returning from
Mars.

I'm sure that NASA will try to do the right thing here and do better than
that. The fire on Mir taught us some lessons too.

Hard to say, but everything worth doing has risk. Even getting out of bed
in the morning involves risk.
