Your list is pretty spot on. Mark Kelly and Jim Kelly are both probably
up for flying as CDR sometime. Of Polansky, Scott Kelly and Steve Frick
I'm sure at least one of them could land another flight as CDR. Same
I'm rather hoping that they *all* could.

Dominic Gorie, a three flight veteran who commanded STS-108 is still
active. I would think that based on the previous flight rotation, he
would be one of the next in line, followed by Altman, Ashby and
Bloomfield.
I remember reading an interview with Jim Kelly after STS-114 in which
it was stated that he would probably be up for command of a future
shuttle mission, but he said he would be willing to give up that
opportunity if it meant being first in line to command the first CEV
flight.
