That's a design decision, _NOT_ a law of nature. There's no
particular reason why a cargo module could not be carried in the same
manner as the LEM. Or that the manned portion could not be the nose
of the whole craft. MOL is an example of the latter. The Soviets
also designed one based on Soyuz using, like the MOL Gemini, a
heatshield hatch to acess the station via the cargo compartment. (The
name escapes me at the moment, Jeff or Greg might recall it.)
