Survey input software question

Hello,

what program would you recommend for basic paper-to-computer data input?
It should have (of course) input range checking and should output
comma separated value lists. The survey woudl be carried out in
developing countries so it would be great if the software were free,
does not need a network connection and runs on low-end PCs.
