You need following:

Polarizer
INKO Condenser (with DIC Filter)
Plan or better Objectives
DIC Slider matching Objectives in Holder (Zeiss)
Analyser for DIC marked II or III in you case

Alignment and set up instruction can i mail you
if you send you email

PS / There will take some time to setup first time and
a Phase telescope will help a lot
